  • OHSAA Physical Form (includes Consent Form)

    This form is to be completed yearly.  If a student athlete participates in more than one sport, only one physical is needed for the academic year.
  • Consent Form

    This form is completed yearly along with the athlete's physical exam.
All of these forms are also available in the athletic office during business hours. Coaches will pass out these forms before the season begins with instructions.
